## Recognition for 22 Professionals Supporting Patient Wellbeing

Hospital de la Santa Creu i Sant Pau is marking International Healthcare Social Worker Day on Friday 3 October 2026, celebrating the vital role of its 22 healthcare social work professionals. The day highlights their contribution in addressing the social dimensions of illness, which can profoundly affect patients, families, and communities. Healthcare Social Work (HSW) at Sant Pau ensures that medical care is delivered with full consideration of a patient’s social context, including family dynamics, financial stability, housing, and community support. These factors can directly influence recovery and long-term health outcomes.

## Integrated Support Across Hospital and Community

The HSW team comprises 19 professionals based at the hospital and three at the Dr. Pi i Molist Community Resource Center, coordinated by the Head of Social Care. At the hospital, they operate across multiple departments including Emergency (with 12-hour daily coverage, 365 days a year), Women and Children, Mental Health and Addictions, Geriatrics, Oncology, Neurology, and Cardiology. At the community centre, the team focuses on rehabilitation, housing support, and social inclusion through individual, family, and group interventions. Their work includes planning safe hospital discharges, offering emotional support, safeguarding patient rights, and assisting caregivers.

> Their interventions help anticipate vulnerability, ensure continuity of care between health and social services, and connect individuals with essential resources.

Approximately 15% of hospitalised patients receive support from HSW staff, while all users of the Dr. Pi i Molist centre’s services benefit from their involvement.

### Commitment to Holistic, Humane Care

Sant Pau recognises the role of healthcare social workers in reducing health inequalities and delivering coordinated, person-centred care. Their efforts bridge clinical treatment with social reality, particularly for those facing poverty, isolation, or violence.
